PREPARING FOR ADMISSION

Admission takes place over a number of days and consists of reviewing your health, personal, and financial information to provide you with the best stay possible. We’ve reviewed a few topics that will help you prepare.

INTERPRETIVE SERVICES

Interpretive services are available to patients for any type of care. They are provided at no cost and are available daily around the clock. Please notify the Charge Nurse, the Director of Social Services, and/or the Executive Director for assistance in obtaining interpreter services.

ADVANCED DIRECTIVES

An advance directive is a written document, such as a living will, instructing the center regarding what kind of care you desire in the event that certain circumstances arise and you are unable to speak or communicate your desires.

RATES & BILLING

Our current Basic Rate includes room and board, routine nursing services and personal care, social services, activities, laundry service, housekeeping, maintenance services, accounting, and billing services. Additional personal expenses such as pharmaceuticals, beautician services, transportation, medical supplies, and other personal need items may not be included in the daily room rate and will be charged as additional expenses.

PAYMENT OPTIONS

We are happy to help you make the most of your benefits and other source of income. While some benefits depend on your income and savings, not all help is linked to your current income. As your needs change, your entitlements may also change. If you prefer not to discuss your financial affairs with our staff, we will be happy to refer you to someone else who can help.

PREPARING FOR DISCHARGE

Preparing for discharge begins upon your admission to our center. First, all residents are evaluated upon admission. Second, we collaborate with you and your family to determine appropriate and attainable goals that take into account your needs and abilities. Third, as you rehab and recover, we help you prepare or life at homes after rehabilitation.

CONTINUED CARE OPTIONS

If discharge home is not an immediate option, there may be other communities that can help you transition to a more independent lifestyle.
